Please refer to page 89 on datasheet for PBTL operation. Yes SPK_INB- and SPK_INB+ are tied to GND for PBTL mode. PIN26 ADR0, is used to set the I2C address. Setting ADR0 to GND and DVDD (3.3V) will set I2C address 1001100x and 1001101x, respectively. To select left or right channel data is done via Page 0 register 42 (P0-R42).

If you need to support 192k with TAS5756M you will have to use HF5 as you stated. If you needed SFS/DDE/DBE you will have use HF2. TAS5756M can't update it's own process flow automatically based on sample rate. It needs the host processor to detect and manage download of HF base on sample rate.
If you add a SRC like SRC4392 to the front of TAS5756M, you can use HF2 for all sample rates since the SRC will can convert all input sample rates to 48k for TAS5756M. There will be no need for changing HFs.

The second from you right. You can use HF2 for the features you want with the PBTL config then use P0-R42 to select L or R (A or B) channel data for the corresponding speakers.
