INA199A2 Output Problem

Thank you for your question. I have couple questions for you. 

What do you mean by In+ and In- are the same. Does that mean that the sense current is zero. Are you also moving common mode? 

From the current plot I calculated a max Vsense of 7.5mV. Is that right?

No, the sense current isn't zero.
The current plot shows the sense current.

Does the green plot above show the common mode? Is that oscillations that I see in the plot? Are the common mode and sense current changing simultaneously?
Also have you tried disconnecting the INA199A2 output from the other circuitry and then measured the output. Have you measured the sense voltage at the device inputs?
