INA219B destroyed?

Hi,

I have an adafruit INA219B connected to an arduino nano. On the measurement side I have connected ~ 16 volts. I use this combination to detect if the battery is charging or discharging. It worked pretty good. Till yesterday.

I am not sure what happened but the 1st pin (Vin+) seems to be broken. The edge of the chip is broken.
Has anyone a suggestion what can cause this? I am not sure about short peak currents when charging but the average is less than 500 mA.

Before I destroy more boards of cround 10€ each, it would be good to know why that happened and/or how to protect the little chip.
