What does '12 V' in this formula mean?

    The 12V in the formula means that the testing conditions for Table 8.5 Electrical Characteristics had the common-mode voltage (Vcm) set at 12V. Offset induced by CMR is due to a change in Vcm so there needs to be a reference point. Since we tested the part at 12V and measured an initial input offset of 5uV, this is the reference point. CMRR was calculated by changing the Vcm from 12V to 80V and 12V to -4V and measuring the additional offset.
