Using LF398 to create a simple sample and hold circuit

I have an on/off signal (could be any voltage 5V, 12V, 24V) that I need to convert to a pulse that is either a negative or positive 24V based on the signal action (turn on = +24V, turn off = -24V). The duration of the pulse is somewhere between 5 sec and 30 secs (it needs to be settable).

The load is a linear actuator that uses about 1A at 24V.

It looks like I need a sample and hold circuit and the LF398 looks right.

I have a 24V source for the energy that the actuator needs. I can use relays based on a signal, but I'd like to use the device to drive the actuator directly if possible.

What is the simplest possible circuit / device to achieve this functionality?
