My customer wrote:
We are using the INA117 in one of our products and we have a small number of very unusual failures.
The differential amplifiers are used to measure individual cell voltages in Nickel-Cadmium batteries where the highest common mode voltage is less than 80V.
In a very small number of cases (out of 3000 amplifiers in service since 2001) we have observed a very high leakage on the front end. This becomes evident when the circuitry is connected to a resistive test circuit (source resistance under 200 ohms) and the amplifier with leakage disturbs the voltage set by the divider.
In two cases, we had the amplifier actually burn up.
Are these isolated cases? Are there any conditions where the amplifier is vulnerable?
