how to get equation 1?

if VCM = GND
VOUT = (R4/(R3+R4))+((R1+R2)/R2)*VOUTP-(R1/R2)VOUTN
if VCM, not GND, why become equation 1? not VOUT = (R4/(R3+R4))+((R1+R2)/R2)*VOUTP-(R1/R2)VOUTN+VCM

Since R1 = R4 and R2 = R3 the equation simplifies to what is shown in the technical document.

"V3" is the voltage at pin 3 of U2.
