For the SMT op amp, take a look at LM7171/LM7372 (single/dual, stable at Av >= +2 or -1) or LM6172 (dual, unity-gain stable). Both of these are VFB op amps that can meet your speed, supply voltage, and SMT requirements. Note that the slew rate for these devices is proportional to the input level, and higher slew rate can be achieved in lower closed-loop gain configurations.
For high output current buffer, you may also consider LMH6321, which has 300 mA output current, adjustable current limit pin (resistor set), and is available in 8-pin PSOP (SOIC with Thermal Pad ) or 7-pin TO-263 SMT packages.
