hello,

             I need to amplify a 4MHz signal with an amplitude of 10mV so that the output varies between 0 to nearly 5 volts. This signal lasts for about 5 micro seconds.I wish to amplify it using a single stage amplifier with single power supply of +5V. My current solution is to use two OPA354 amplifiers. It seems to work in TINA-TI, however if possible I would like to do the same with a single amplifier. 

Can any one please suggest an amplifier that can achieve this task?

    Given that you want a 5V output signal, this will require an amplifier with a good "Large Signal Bandwidth". The OPA695 should be able to meet your needs. Note that the OPA695 is a current feedback amplifier so, the larger the value of Rf that you use, the more the BW is reduced. Please go through the datasheet and also simulate it in TINA to ensure it meets your requirements.
