Isolation Amplifier Power Supply

Hello,

This is a question on TI part ISO124, which is an Isolation Amplifier.

I never used it before, but the data sheet states that the power supply voltage is +/-4.5V Min and +/-18V Max.

1. Do I need both plus and minus power supply (+Vs1 and -Vs1), if my input signal is +1.5V and changes with time to 0.2V Min? May I ground -Vs1?

2. Do I need +Vs2 and -Vs2, if I am expecting +1.5V at the output, or I can put   -Vs2  to GND? (Gain is 1 in accordance with data sheet).

3. May I use different voltages, e.g. Vs1=+5V, Vs2=-15V?
