XTR111 Exploding

I'm having a trouble with this chip. I've made a test board and every thing works well. In my final board I'm having a trouble. If I connect the board to my Source unplugged, and energize the circuit everything works well. But when I "hot-plug" the chip burn out, near to pin 2-3. I'm using the same circuit sugested in datasheet. To output I'm using IRF9530 and BC857.

    Please provide a complete schematic for your system so we can take a look at the connections.  Since you mention the BC857 it sounds like you've implemented one of the external current limiting circuits but we need to see the schematic to understand what you've designed.  Also please describe what connections you're making during your "hot-plug" testing.

    Schematic connections aside, the capacitance of the IRF9530 is over 2x higher than the devices recommended in the XTR111 datasheet and may not be the best choice for this application.  The IRF9530 is a fairly strong PMOS device in a large TO-220 package.  It supports dc currents up to 14A and Vds voltages up to 100V which are both well outside of what is required for this application.  If you can't find or don't prefer to use the PMOS devices suggested in the XTR111 datasheet, consider the BSP170P which has worked well in many applications.
