PGA113: how to config PGA113?

I can confirm that your first image does appear to be sending 0x2A11, and that that register configuration should connect channel 1 with a gain of 2x. What was the issue you were experiencing with the device?

According to datasheet, "SDI" signal is sampled on the rising edge of "SCK" while "CS" is low. so I think, I am sending 0x2A11. its doesn't output anything. its output is a constant.

I finally programmed it. but its output is like below image. I don't know why its output is corrupted. when I reprogram it, its outputs is very nice but after a short time its output becomes invalid..

I'm so sorry. I forgot to answer you.
first I connected the input signal to ch1 (ch0 was connected to gnd). after a random time it was switched to ch0. to confirm this I connected another signal to ch0 and I saw that the signal which was connected to ch0 appeared at output.
I connected the input signal to ch0 and it work well.
I don't know why this is happening. maybe my IC is damaged.

To confirm, you see the expected output from the PGA113 if CH0 is selected, but CH1 does not work as expected? Is there any capacitive load on the output of the PGA113? Have you tried an A-B-A swap of the PGA113 (swap the suspect unit for a new unit, re-test, then swap the original unit back in and re-test again)? 

What other connections are made to the output of the THS4031?
