Part Number: TLV320DAC3101
Our customer's board uses the device TLV320DAC3101IRHBT and finds about 20% failure on first power-up (no audio). The device is replaced, and continuous to function as required. What can cause the device to fail?
Some unused inputs are not tied (left open), i.e. AIN1, AIN2, VOL/MICDET, can this be a cause of failure?

Hi Ivan, thanks for your reply. Once the unit fails, power-cycle does not recover the device. Damage seems to be permanent.
Attached are plots of power supplies with respect to RESET signal.
