TINA/Spice/DRV110: Peak level not correct and no switch to hold

I have a simulation I am working on involving the DRV110 and I have an issue where the PWM output is not switching to hold levels as expected.  It is also not putting out what I would expect for peak levels.  The current is consistently rising higher than expected for the resistor values. The schematic and waveform are below. Could someone take a look and tell me if they can explain why I'm not seeing a switch to hold and my peak levels aren't as expected? I have this circuit made up as a prototype as well and it functions as I would expect it to in physical hardware.
