LSF Family Eval Board Question

I recently purchased the LSF family eval board and I'm having some trouble getting it to perform correctly. I thought as a good first test, i would build the circuit in the application notes (attached, for reference) Figure 2 in document SLVA675B. I have it set up on the bench EXACTLY as shown, with the exception that I am using 3.3V on the A-Side and 5V on the B-Side. Im sending through a square wave (tried it at a variety of frequencies from 1khz up to my crappy wonderful signal generator's limit of 1MHz. When the wave is high (3.3V on the A-Side, my B-Side is at 5V. Perfect. However, when the wave goes to zero, both sides only fall to ~2.8V (see attached scope screenshot). I cannot figure out why this is the case. Am i being dumb and forgetting something? Is my signal generator not strong enough to pull the A side down to zero?
