Question about SN75176B A output swing

Hi,

My customer is now evaluating SN75176B,

and some of SN75176Bs have trouble with pin6(A) output.

As seen in the attached wave form.( A out put swing got lower)

the phenomenon went away when we replaced SN75176B to new one.

Have someone ever seen something like this before?

Did the IC get damaged?
