ISO 1540 burns out

Hi

I am using i2c isolator ISO 1540 from TI. We are using it by giving 5V on one side and 3.3 V on other side. Capacitors are also attached as recommended. But every time, it works fine for 5 10 seconds then heats up and burns out. It has happened 10 15 times already. I have attached schematic of its use as well. Please replay and help ASAP that what are we doing wrong??
