TPS65982: Stuck in BOOT mode

Hi,

I have programmed and verified my new TPS65982 device (custom board) using both the Snapshot and Aardvark Flash Center GUIs. When I download the programming from Flash center I am able to see the contents written on the flash memory (using the same memory that came with the eval board). Here is a screenshot of my program download:

The issue is that I am stuck in BOOT mode and never move into APP mode:

Boot flags are also showing as bad:
