LSF0102 Level-Translation Problem

What devices are driving the signals to the A1/A2/B1/B2 pins? How much current can they handle, at what voltage drop?

You did not answer any of my questions.

Anyway, the measurements indicate that the device driving MDM_DBG_UART_TX cannot handle 10 mA.

But it is not necessary to use 10 mA (assuming all devices are CMOS); you could just as well use 0.1 mA.
Furthermore, it appears your devices have push-pull outputs, so you do not need the pull-up resistors on the LSF's inputs.

So remove R5 and R13, and increase R6 and R12.

It might have been easier to use unidirectial level translators, e.g., 2× SN74LV1T34.

The low side (typically A-side) is used as an output or a bidirectional port, is translating to Vref_A, and there are signal integrity problems.

What is thesignal integrity problems?
