Regarding SN74AVCH8T245:

Thermal PAD at PACKAGE OUTLINE(page 28) have number 21 .

Termal PAD at EXAMPLE BOARD LAYOUT(page 29) have number 25 .

Termal PAD have number 25 and should be connected to GND?

The 'normal' pads already have numbers 1…24. The label "21" must be a copy/paste error left over from a smaller package.
