Concerto Error: Emulator Disconnecting

I am getting errors 100% of the time while trying to debug with CCS 5.5.0.00077. The problem is, it seems to be a different error every time. Also,  I am using a controlCard with a Rev B part.

So far, I have restarted the PC, removed all other USB devices that use COM ports, removed and reseated the DSP card, sacrificed a goat, and even tried to use the Blinky test project.

EDIT: This issue seems to have started when I installed another device that uses FTDI drivers... is it possible that we have a driver issue? How would I check/solve this? The under "Ports" in Windows 7,  Driver information is as follows:

    Just spent the day on this.

    Even tried uninstalling and re-installing CCS, lord have mercy that took forever.

    Here is more detail on the error  (there are no breakpoints set right now):

    Cortex_M3_0: Please make sure the memory location you are programming have not already been programmed.
    Cortex_M3_0: Trouble Setting Breakpoint with the Action "Finish Auto Run" at 0x201464: (Error -1066 @ 0x201464) Unable to set/clear requested breakpoint. Verify that the breakpoint address is in valid memory. (Emulation package 5.1.232.0)
    Cortex_M3_0: Trouble Setting Breakpoint with the Action "Terminate Program Execution" at 0x2017a8: (Error -1066 @ 0x2017A8) Unable to set/clear requested breakpoint. Verify that the breakpoint address is in valid memory. (Emulation package 5.1.232.0)

    EDIT: This issue is definitely limited to the Concerto processor.  I just tried another controlCard (28069) without an issue.

    EDIT2: I tried Uniflash and was able to successfully delete all the sectors but the problem remains.  The M3 constantly states that it is "In Reset"
