I am desinging filter using UAF42 to design a 60 Hz Notch Filter and following the origional design application given at http://focus.ti.com/lit/an/sbfa012/sbfa012.pdf
But i am facing a problem that the shape of origional waveform is also changed while filtering.
Kindly guide me what could be the problem. Pictures of the results are attached in the attchment.

This 60Hz notch filter should be well behaved and what you are seeing is definitely not right.. I have attached your circuit with a simulation done in TINA.
It looks like there is another frequency being mixed in with your test frequency-- could your circuit be oscillating? That makes me wonder if perhaps the "Q" is set too high. The value on the circuit diagram is OK-- could that resistor be wromg? Another thing to check is your bypass capacitor-- you should have something like a 0.1uF ceramic capacitor connected from each supply pin to ground.
