DRV2700EVM: My DRV2700EVM board has apparently died

I have a new DRV2700EVM board.

I was able to successfully operate it with the USB GUI software supplied with a piezo actuator connected.

Then I made the following jumper changes and connected to an external 5V bench supply and signal source:

Remove JP5 and JP6

Move JP10 and JP11 to VIN positions

Move JP7, JP8 and JP9 to pull-up position

Connect a 5V bench supply to J2

Connect a signal generator to J5+ set to 100mV sine wave at 500Hz (ground to board GND)

I did not see any significant output from the DRV2700.  I started to investigate and found that there is no 3.3V power (voltage across C2 is about 0.5V).

I have been careful and worked under proper anti-static conditions.  The power supply never entered an over-current condition.

I believe the board has failed due to a defect.  Can I get a replacement?
