We have used BQ24610RGET battery charger IC in our module,we noticed that the LED connected to PG pin of the IC is Blinking on providing Adapter I/P supply, even though Adapter supply seems to be good and we confirmed the same using a second board, where we found it functioning as expected.
This was not observed initially, we are suspecting battery was connected with wrong polarity during testing ,which is causing this faulty behavior.
We too noticed no battery charge voltage is generated by IC at battery connection point.
Whether connecting battery with wrong polarity will cause failure of IC or something else is causing the issue.
Plugging in the Battery with wrong polarity can damage the IC. Please replace the IC and check if all the FETs are good and test again.

Thanks for the reply, we probed the ACDRV pin connected to gate of input side P Mosfets and found it constantly switching its state at interval of around 50mS ,before replacing the IC we would like to confirm anything else will cause this observation ,since the criteria for Turn ON/OFF of input side P Mosfets is comparison of voltage at SRN and VCC Pins, which seems to be fine in our case.

We have seen waveforms at VCC ,SRN and ACN pins and found VCC pin is dipping below SRN voltage at regular interval(ACN wrt SRN is normal ),we are not sure on the reason behind this dip in the VCC voltage, any clue could help us to make proper decision for replacement of IC if damaged.
We can't make sure that the IC is damaged , since most of its features seems to work and doubt that the problem will persist with new IC also.
We have verified other discrete components connected to the circuits and everything is fine.

Yellow:VCC Green :Battery Voltage
VCC dipping at the interval of around every 1.5Sec
You can try add more capacitor at the input to see if it can hold up the VCC.
