ground and thermal pad connection in lm5045 PCB layout

I have some problem in two layer PCB layout design for LM5045:

first, should the thermal pad under the IC connected to ground?

and  second, in datasheet it's said that power ground and analog ground should be connected directly under the IC. does it mean they must be connected to thermal pad and all the three must be grounded?

any help would be appreciated.
