UCC28180EVM-573: The reason for FET damage

Customer is evaluating UCC28180EVM-573 for below condition, but when output power increased No load to 80W, FET was damaged.

Please let me know about possibility of the reason for FET break.

・Vin:85V

・Pout:No load to increase 80W

・Evaluating condition are the same as User's Guide and default setting. 

 One only difference, Vcc is 15V from external DC power supply. (User's Guide is Vcc:12V)

    There should be no reason why this failure should occur. Can you order another EVM for the customer ?

    Thank you for quick reply,

    Customer have EVM only one board, and they will replace broken parts and evaluate again.

    Updated information, broken parts are switching FET and gate Diode, picture is attached below.

    This picture look like insulation failure, because of Ark marks and broken insulation sheet.

    I think that if screw clamp is tight, insulation sheet become thin and decrease insulation voltage, it is one of reason.

    Is this correct? and, is there the other reason?

    I think the must have been some breakdown with the insulation either due to mechanical stress or some particle getting in between the insulator and the device.

    We can offer the customer a free EVM if this helps.

    Already new EVM was ordered by TI Japan.

    Near future, I will support this EVM.
