TPS65150: Can't start up when power on and ff in a short time

In my design, if I power on and off in a short time, there is a small probability that TPS65150 can't start up normally.

The follow picture shows when the issue occurs the waveform on the SW pin. There is no PWM out from SW PIN.

The follow is our schematic diagram, please help to check whether there is mistake in it and give some advice on the issue.

    Can you please give an estimation of how fast you are turning on and off the device? And what do you mean with small probability (how many boards did you test and how many did have this problem)?

    Also do you have a picture of the input voltage when you have the issue (measured on the input pin and on C45)?

    Thanks a lot for your help and the test result in detail as below:

    1、The probability of the problem:

    Their is not a fixed probability, sometimes it occurs in less than 10 times, sometimes it doesn't occur in 100 times.

    2、The switching frequency

    Turn on and off the device About 500ms 1 time, and the problem occurs. (And there is a regular phenomenon that when the problem occurs, every time when I turn on and off, all the output voltage rises a little, after a few times the device can't start up)

    3、the waveform of the input voltage as below. Yellow Line: Input voltage(C45); Green Line: Voltage on C32; Blue Line: Voltage of Pin VGH

    4、two Strange phenomenon

        a、If I weld the C47 on the DRVP but not SW, the problem doesn't occur any more

        b、I found the voltage on the pin FBP is 0.1V higher than normal, so the  output of  pin VGH is 2V higher, there is no PWM output on DRVP. But all the other output voltage(VGL VCOM AVDD) is noraml.

    I tried to reproduce the issue on the TPS65150 EVM board but the outputs remain ok as expectced with the VIN profile you provided. Can you repeat your tests while you bypass the "disconnect" circuit at te input I circled in red below. What is the motivation for adding it?
