TPS54232 Failure - Input Overvurrent

Is there any one thing that is more likely cause a failure where the VIN pin, Pin2, will burn through the plastic chip body?

It appears that when the chip failed if shorted the +24V input voltage through to the output and burned out most of the chips tied to the output.  The damaged chips then shorted the regulator output to ground.

The board was laid out per the suggested layout in the data sheet.

Overvoltage possibly??  We are running on a 24 volt automotive battery powered system and at some times the operating voltage may approach 28V.

This has happened with 2 out of 10 boards with the same results.  There are brushless motor drivers on the board so there if probably some noise in the intput voltage.  The motor drivers are also laid out to chip mfgrs recommendations and there is a lot of ground plane.
