TPS65916: GPIO_5 used as POWERHOLD is being held low on startup

We are using the TPS65916 PMIC to power the AM571x processor.  On power-up, we are using an external regulator's PGOOD to enable the PMIC using GPIO_5 (POWERHOLD).  For some reason this line is being held low (at approximately 0.5V) when we expect it to be pulled up to 1.8V.  Is there anything else that needs to be done in order to use the POWERHOLD capabilities or any reason we should have an issue using an external regulator to enable it?

    You should not need any other steps to bring POWERHOLD high and enable the TPS65916. Can you check the requirements of the external regulator? Does it's PGOOD signal require a pull up resistor?
