PTH12050WAD - part over heating

We draw less than an amp from PTH12050WAD and all within spec.  It over heats and thermally shuts down.  Do you have any suggestions on how to resolve?  Currently, we have chassis' open and a large fan directly blowing on part so we can continue with development.

    What is the  output voltage set point? Send a schematic of your circuit with input and output capacitors ,capacitor  ESR rating  and total output capacitance. How are  the tracking pin and inhibit pin used or are they open?

    How are the through hole unit  power pins soldered onto a 100mm x100 mm 1 ounce copper plating surface?

    Note: PTH12050 inductor when  vout is set to 5V soldered onto a 100mmm x100mm copper area temperature at 1 amp is about 90°C.
