DC to DC Converters Isolated at and above 4200Vrms.

I've been asked to isolate our external USB at 4200Vrms.  

I have a 12 volt input and I need to get it down to a 5 volt 1.2 amp output.

My problem here is finding a transformer to do the job.  There are plenty of DC to DC converters on the market, but I can't find a transformer that works.
