LMZ21700: IC failure in field

Hello,

We use this IC in an automotive application to convert a +12V battery rail to 1.8V and have observed a failure. The output impedance between pin 4 and ground is 4 ohms and the output voltage is zero.  Are you aware of any conditions that can cause this failure. There was no other failure on the PCB and the current draw and performance returned to normal when this IC was bypassed.
