TPS4H160-Q1: Thermal Shutdown Threshould

Dear Specialists,

My customer is evaluating TPS4H160-Q1 and has a problem.

I would be grateful if you could advise.

Operating condition

the auto-retry mode. 

VS=24V,

Every output is connected 4.3kohm, this means 5.6mA of output current each.

  For the first time I confirm the operation, thermal shutdown was active at 100deg  package surface temperature.

  And then I restart,  thermal shutdown was active at 75deg package surface.

 Keep operating, the device couldn't output at certain channel.

question

(1)It seems to be changed thermal shutdown threshold, I'd like to know why.

(2)Power dissipation is not so much, I'd like to know why the thermal shutdown is active.

(3)I'd like to know are there any other possible causes.

For example, thermal pad connection is not enough,  can this phenomenon occur.
