TPS25944L: Damage during Hot-Swap

Hi, Team

My communication customer is using TPS25944L in their BBU system. There is 12V on back-plane. TPS25944L is used for hot-swap and multiple protection on system board when the board is inserted to back-plane.

Customer find TPS25944L damage during hot-swap test (keep remove and insert the system board to back-plane).

Did you see similar damage before? Could you help to analyze the possible reason?

Here is the waveform after TPS25944L is damage.

IN pin is keep stable 12V. EV/UVLO, OVP voltage keeps correct.

When MCU find TPS25944L output is not correct, it will pull down the EN and pull up again. The OUT pint will rise up to about 6V and then drop back to 0V.
