What is the process of requesting a CMRT from Texas Instruments?

This is the process for requesting conflict minerals activity at Texas Instruments.
To request the latest version of the CMRT template, please use this link, Email TI , to Email the Customer Support Center with your list of TI part number(s).TI will respond within 1-2 business days.
You will need to be logged into your myTI account to access the Email form.
Issue Type; select “Quality, Reliability, Environmental”
Company Name; provide the name of the requesting company
Issue description; provide a list of the complete TI device part numbers
Attach File; you may attach an excel or other file with the part numbers.
