1. OFFSET_A is around 2.6V at 000h with dual supply. When it's 800h, OFFSET_A was 0V.

we observe the DAC7718 heating up to 45 degrees C when the negative -5V voltage is applied to AVss (through a 1kOhm resistor). The AVss is taking 2.5mA, its voltage is -2.5V.

we've observed oscillations on the reference input of the DAC (in amplitude, around 100mV, few tens of kHz). Removing the 1uF capacitor in the Vref, the oscillations were gone.
