Just to note this issue is still ongoing.
We have now seen 3 more failures making 6 so far in last few months.
We unfortunately didn't record the info earlier but the latest 3 chip markings are

My customer is using LM2854 and they use tss =approx 1sec.
Therefore, Css is around 14uF (10uF+4.7uF)
Is this accpeptable?
Would you please let me know the max. value for Css?
