As Clemens said, it's possible that you've damaged the device. The OPA990 output (TLV1822-Q1 input) can swing negative such that it goes beyond the absolute maximum ratings of the device.
The LM2904V does not have rail-to-rail inputs; the input common-mode voltage range ends 2 V below the positive supply. Use something like the TL081H or OPA990.
