The PTH08000W can be overstressed if the inhibit pin does not have a dedicated open drain FET for controlling on-off function. The unit most likely has a short on the Vout bus to ground.

The PTH08000W must have a dedicated inhibit pin control FET for each module.
The inhibit pins must not be connected together. The PTH modules have their own internal pull-up. Do not place an external pull-up to this pin.
