Use the EC table for 10 ppm/C for SOIC8. The REF50 used to be offered in MSSOP and that package had a 5ppm/C LTD after 1khr. The datasheet needs to be updated to reflect the current LTD.

You're right, REF5045 is not mentioned for some reason. I'll look into this offline, but meanwhile, please consult the REF50-Q1 datasheet , where you can see that the typical will be 4.5 V as you mentioned.
