Does the Class D output have any protection? The sound from the speaker is stopped (no PWM) when the loud signal is played with high gain. The high gain setting is necessary to achieve clipping below 0dBfs.

Use soft-stepping in your volume control as a start, refer to datasheet search under pop for more information.
There is also page 0 register 42 for output driver power delay that can help reduce pop.
