    I think I know what the issue is. I'm using the TPS40211-Q1. The design guide I followed is for the TPS40210-Q1. The TPS40211-Q1 ref voltage is 260mV. The TPS40210-Q1 ref voltage is 700mV. I will correct for this and report back with results.
