I tried also with an TPS40210EVM (evaluation board from TI) and I found the same behaviour.
With the same load (an wireless AP) with 8-12 Vin voltage.
The TPS40210 goes in OC every 700 ms !!!

The DGQ is probably the easiest of the two packages to solder. The best I can recommend is to keep on trying to clean off the solder shorting any of the pins together.
You could also order the TPS40210EVM for prototyping purposes.
