The TPS54540-Q1 is more sensitive to noise than the TPS54540B-Q1. Since the part is not shutting down due to heat, noise is the most likely problem. Three approaches to reducing noise sensitivity are:
1) Use the TPS54540B-Q1.

The 0.1uF cap super close to the chip does help a little bit with noise, but doesn't fix the problem.
I will have a look out for the TPS54540B and see if this fixes the problem.

The TPS54540 is not recommended for new designs.
I am reviewing your schematic now, but I believe that may be related to your load regulation issue.
