The TPS62480 was probably released before we started adding this to our D/S.
As well, for a newer, smaller, cheaper 6A device please look at TPS62866 and TPS62869. Would these work for your customer?

Did you have a load attached when you applied Vin?
Can you send a photo of your PCB?
Connecting FB to Vout should just force the output voltage to be 0.6V. I am curious how this connection occurred on your PCB.
