I believe the main difference between these two parts is their ESD rating. TRS232E has a much higher rating at 15kv HBM and specifies ratings for contact and air-gap discharge. It also appears that TRS232E has extra packaging options.

TRS3223-Q1 unloaded DOUT is nearly the same as loaded because the charge pump is regulated.
The TRS202 and TRS232E have higher DOUT because the 5V charge pump is unregulated for close to double voltage.

TRS232E and TRS202E have pull up resistors.
The data sheet states this feature as Driver input current. Edit: TRS202E does not have pull up resistors.

Do they all work up to 6.5Vcc or just MAX232?

Even with 5V supply, the Voh might just be similar. The reason is that the abs max of V+/V- pin (charge pump output) is 7V. If possible, you could consider
