Is your circuit a flyback - if so, it looks the transformer polarities are not correct. If it is forward then the secondary side should have the output inductor. If this is some different topology, please describe and explain how it can work.

You would need an external amplifier and some other circuitry to get the LM5021 to operate in a non isolated flyback.
