Any input voltage less than 0.6V will result in the ULN2003LV output being off.
A 0.45V (max) input is fully compatible with the ULN2003LV input low requirement.

Can you replace the ULN2003LV or try a different channel? A waveform of the output trying to turn off would also be helpful. The results are very unusual unless the output is permanently damaged in some way.
