We found root problem for our issue.
We had soldering problem between NTC network and BQ24251 device.
Now BQ24251 is working properly without TS fault on the Standalone mode.

i have meet the same question on bq25120 , sometimes when plug the battery(3.8V), there is no output . and i measured the PMID ,the PMID voltage is just about 0.3V。
