From a first look your schematic looks good.
Maybe the MOSFET only being 3mm x 3mm might get thermally warm, consider the power loss and heat management of those components.
Please double check your component values with the quickstart calculator…

At first glance this looks like compensation instability.
Can you fill out a LM25145DESIGN-CALC with your values and attach it here?
If it is not the compensation values then it may be layout related.

Also the SW node waveform would be very useful.
I think your schematic might be ok as it starts up to 12V, but there is definitely some instability I think is layout…
